International Medical School Graduate (IMG) Physicians

Securing Residency, J-1 Visa, Conrad 30 Wavier, non-Cap H1B Visa, Job Offer in the USA

In order to secure U.S. medical licensure and a job as a U.S. licensed physician in a U.S. hospital or private medical practice, International Medical Graduate (IMG) physicians must complete a U.S. medical residency and undertake the following steps if the IMG physician wishes to practice in the United States. It is important to note that IMG physicians at US & Canadian medical schools must follow similar steps.

  1. Take the United States Medical Licensing Examination (USMLE) Step 1

The USMLE Step 1 is a one-day multiple-choice examination that evaluates an IMG physician’s understanding of the preclinical sciences needed to practice medicine. IMG physicians must check for testing locations at:  http://www.ecfmg.org/fees/usmle-surcharge.html.  Prep study courses and study books are available on line.

  1. Take the United States Medical Licensing Examination Step 2 Clinical Knowledge (USMLE Step 2 CK)

The USMLE Step 2 CK is a one-day multiple-choice exam that evaluates a IMG physician’s applied clinical knowledge in providing supervised patient care. IMG physicians must check for testing locations at:  http://www.ecfmg.org/fees/usmle-surcharge.html.  Prep study courses and study books are available on line.

  1. Take the United States Medical Licensing Examination Step 2 Clinical Skills (USMLE Step 2 CS)

The USMLE Step 2 CS is a one-day pass/fail assessment that uses simulated patients to measure the student’s history taking, examination, and investigation skills. The test also evaluates the examinee’s communication and interpersonal skills and their proficiency in spoken English. The USMLE Step 2 CS is taken at centers located throughout the United States.  Travel to the U.S. to test will require that the IMG physicians interested in testing obtain a B-1 business visa at:  https://www.usimmigrationsupport.org/b1-business-visa.html   Prep study courses and study books are available on line.

  1. Enter the National Residency Matching Program (NRMP)

IMG physicians enter the National Residency Matching Program (NRMP). This is the system used to match medical IMG physicians to residency 28,000 first year residencies which were offered across the US.   All US residencies are available for review in the online database called the Fellowship and Residency Electronic Interactive Database (FREIDA). When applying for a residency IMG physicians use a single online application called the Electronic Residency Application Service (ERAS) to apply to multiple residencies.

  1. Conduct Interviews at Residency Programs

Around December IMG physicians receive interview invitations from US residency programs.

Completing Residency in the USA

  1. Take the United States Medical Licensing Examination (USMLE) Step 3

After residency training has commenced, the resident arranges to write the USMLE Step 3, which assesses a resident’s ability to apply clinical knowledge to the unsupervised care of patients.

  1. Take the Specialty Examination

Near the end of residency, the board examination for the resident’s chosen specialty is taken. For example, a family physician would take the American Board of Family Medicine (ABFM) certification examination.

For a fee, payable incrementally AFTER the completion of progress milestones detailed herein Icon will assist qualified IMG physicians to obtain a job offer conditional upon their admission to, and successful completion of, a U.S. medical residency program; assistance with admission to medical residency program (and J-1 visa); after residency, to interview for and obtain a non-conditional job at a hiring hospital; referral of hospital employer and IMG physicians to a competent visa and immigration law firm for the filing for and issuance of a non-cap H1B visa; and, assistance and successful issuance of state medical licensure for the state of employment. Services mentioned here may be completed through the services of Icon sub-contractors.

We place to all states, however we targeted these rural states for our IMG Employment program, due to better ease of obtaining Conrad 30 waivers:

Iowa Nevada Utah

These are the states with the most need for physicians.  After the IMG completes his / her medical residence in America, they work for a hospital in one of these states on a 3-year contract period and then move on to any state in America.  Mostly I am looking for primary care physicians including family medicine, internal medicine, pediatrics, gynecology, obstetrics, pulmonology and ER .. all recognized hospitalist specialties.
